ALL 6 (six) FORMS NEED TO BE FILLED OUT COMPLETELY AND TURNED INTO THE SCHOOL OFFICE BEFORE YOUR CHILD CANSTART CHILD CARE.
Philosophy: St. Damian's Child Care program is designed to provide a safe environment with activities for school-age children before and after school.
Morning Care: Monday - Friday 7:00 a.m. - 8:00 a.m.
$6.00 an hour or $3.00 per half hour
Afternoon Care: Monday - Friday After School 3:00 p.m. - 5:30 p.m.
$6.00 an hour or $3.00 per half hour
Proximity Card - (key card) The card will let you in the back doors by the library. Entry to the building using the doors in the back. Look for the Latchkey sign on the back fence of the parking lot.
Program Plan and Schedule for Morning Care: Children in the Morning care room may choose from a variety of games and use of the computer. A Child care provider supervises the children during indoor play When the bell rings at 8:00 a.m. the children walk down to their classroom to start their school day.
Program Plan and Schedule for Afternoon Care: After school the children walk from their classrooms to the after school room. The child care providers check in each child. Children are given time to eat their nutritional snacks and change into their play clothes. Child care providers plan and supervise children participating in many activities. These activities include outdoor play, homework and free choice of materials.
Our After School Program offers a playground with a slide, swings, and a playscape. The children are able to play with various playground equipment and use balls, hula hoops and jump ropes.

To attend before and/or after school care your child’s personal file will need to include:

$30.00 one child or $50.00 per family

_____ Emergency Card – fill out both sides and all spaces need to be filled in. ****Fill out an Emergency card for each child.****

_____ Health Appraisal Form (Physical Form) current and signed by a doctor (get the green sheet from your doctor or school office)
OR a signed statement from the parent stating that (a) your child is in good health with activity restrictions noted.
(b) your child’s immunizations are up-to-date.
(c) the immunization record or appropriate waiver is on file with the child’s school.
